「fail」の過去形・過去分詞。誰かがうまくいかなかったり、期待されたことができなかったり、試験に合格しなかったときに使います。

Usage Notes (Japanese)

「failed the test（試験に落ちた）」「failed to do（～できなかった）」「the plan failed（計画が失敗した）」などの使い方が多いです。カジュアルに言う場合は、やわらかい表現も使われます。
