「トップの生徒」は試験などで最も成績が良い学生のこと、「ケーキトッパー」はケーキの上に飾る装飾のことです。

Usage Notes (Japanese)

文脈によって学生か飾りか区別します。「class toppers」は成績上位者、「cake toppers」はケーキの飾りです。帽子の意味では使いません。
