この表現は、誰かが非常識だったり、無茶なことをした時に使います。とても驚いたり呆れた時に言います。

カジュアルな表現で、親しい間柄で冗談っぽく使うことが多いですが、怒って言うと失礼になる場合もあります。『正気？』『頭おかしい』もよく使われます。
