This is an old-fashioned, playful gesture. Now used mostly figuratively to mean showing disrespect or openly defying authority. Rarely used literally in modern times. Don't confuse with 'give the finger' (much ruder). Commonly followed by 'at someone/something': 'thumb your nose at tradition'.
