Very common in instructions and warnings: 'Hurry up, otherwise we'll miss the train.' It often appears between two clauses, but can also mean 'except for that' ('The hotel was good but otherwise expensive' is wrong; say 'The hotel was expensive, but otherwise good'). Another use is 'can't/couldn't do otherwise' = had no other choice.
